Everyday Purchases... Extraordinary Results

Every time you shop at Safeway or Albertson's, a percentage of your purchase can be rebated back - automatically - to help support the programs and services of the Epilepsy Foundation of Arizona. These funds really make a difference in helping to fund programs such as:

• Social and Networking groups
• Outreach to Hispanic communities, the Navajo Nation and Rural areas
• Information and education on seizure management to schools
• Advocacy efforts
• Camp Candlelight for children 8 to 15 years old.

Safeway Customers:  Complete the information below and we will register you for the program. Then, all you need to do is shop and use your card at the register when you check out. The rest happens automatically!

Albertson's Customers:  Contact Epilepsy Foundation of Arizona at 602-406-3581 or efaz@chw.edu to request your Community Partners card.  Once you recieve your card all you do is shop and swipe your card at the register when you check out.

Visa Cards:  Many valley merchants and restaurants have agreeded to donate a portion of your purchase to the Epilepsy Foundation of Arizona.  Go to www.escrip.com and register your VISA card, then when you eat or shop at any of the participating merchants a rebate check is automatically sent directly to the Epilepsy Foundation of Arizona.
