Please fill out the form below completely.  After you are finished click the submit button.  You will be asked to confirm your information - if everything is correct click the "Make A Donation" link. You will be redirected to a secure PayPal site. You do not need to have a PayPal account to donate online. The Epilepsy Foundation will not receive your credit card information, only a payment confirmation number.  After your payment has been received your name and donation amount will be added to the list of supporters for the participant that you are supporting.  If you select to be listed as an anonomous donor only your donation amount, and city will be listed.
