TITLE: Independent Living Skills Trainer, Syracuse
QUALIFICATIONS:
Bachelor's degree in human services preferred +2 years experience working with individuals with TBI. A valid New York State driver's license and minimum age of 21 is required. Candidates with a combination of relevant work experience and education will be considered for this position. Previous experience conducting functional assessments strongly preferred.
HOURS: Part time, Days and some evenings
DESCRIPTION:
Work one-on-one with individuals with traumatic brain injury to improve their abilities in order to live as independently as possible. Assess individual strengths and areas for improvements. Develop a plan of strategies that profile the desired outcomes of the individual. Provide living skills training in familiar settings or as desired by the individual. Must have reliable transportation and prepared to travel in several counties around the Syracuse region.
TITLE: Community Integration Counselor
Design a plan of service to assist the consumer to more effectively manage and overcome the difficulties and stresses confronted by an individual with TBI living in the community. Part time position.
QUALIFICATIONS:
Licensed Psychiatrist or Psychologist, Masters of Social Work, Psychology or Counseling Psychology or Certified Rehabilitation Counselor. 2+ years experience providing adjustment related counseling to individuals with TBI and their families. A valid NYS Driver's License is required.

TITLE: Community Support Specialist
QUALIFICATIONS: High School Diploma or equivalency with 1 year experience in DD field. Valid New York Driver's License required. Full time.
DESCRIPTION: Responsible for providing residential habilitation services to individuals who are living in the community. This may include individuals who live with their family or who live independently.
Collaborate with the Service Coordinator in preparing a Residential Habilitation Plan within 30 days of start of service. Provide support with interpersonal relationships and assist the individual in accessing and participating in community activities. Maintain service notes for each individual in order to provide documentation that services have been delivered in accordance with the Residential Habilitation Plan.
