Case Management & Service Coordination

The Epilepsy Foundation offers a variety of service coordination and case management programs.  Some services require certification/eligibility of the individual by Medicaid and/or OMRDD.  Other services do not require such certification.

Epilepsy Foundation is committed to serving individuals and their families; we'll work closely with you before, during, and after any eligibility process to find a way to provide you with services.

Medicaid Service Coordination/Case Management for Children and Adults       Available in Rochester, Syracuse & Binghamton

Assistance is given to individuals as well as to families who are caring for a child or adult with a developmental disability and epilepsy or other neurological impairment.  Staff assist in identifying and obtaining needed community services to help an individual and/or help a family that is caring for a family member in their home.  EF staff can address such issues as clinical services, financial assistance, primary health care, and housing (including arrangements for supervised, supportive and other housing arrangements).

Community Habilitation      Available only in Rochester

Provides on-going support and guidance to individuals who reside in their own housing yet who need some support and assistance.  The habilitation specialist works with the individual to help them maintain services (medical, dental, prescription) and to help/assist/improve with some activities of daily living (grocery shopping, cooking, housekeeping). 

STRESSpite     Available only in Rochester

STRESSpite offers families the opportunity to create their own respite in order to relieve some of the daily stresses of caring for a child with special needs.

Family Reimbursement Program   Available in Rochester & Syracuse

Offers financial reimbursement for goods or services to families for a one-time purchase or to assist them in meeting on-going needs for a child with epilepsy and/or other developmental disabilities.
