Supportive Apartment Program

The Epilespy Foundation's Supportive Apartment Program is a transitional community-based residential program for people with epilepsy, designed to prepare them for independent living. The program consists of 14 apartments which are home to 23 individuals. These one and two bedroom apartments are scattered throughout a local apartment complex in Rochester, and are located on a bus route. The apartment complex has a large community room as well as a swimming pool, exercise room, tennis courts, and playground.

Goals

  • To provide a semi-independent living arrangement for persons with a seizure disorder who do not require 24 hour supervision, but who are not presently capable of total independent living.
  • To increase the individual's skills and self-confidence, so that he/she may live as independently as possible.
  • To increase the individual's knowledge of epilepsy, ability to cope with the disorder, and enhance his/her self-esteem through information sessions, support groups, training, and continuing staff support.

Eligibility

  • Be 18 years or older.
  • Have a medically-confirmed diagnosis of a developmental disability prior to age 22 and epilepsy.
  • Be self-medicating and require no more than three hours of support per day.
  • Participate in a volunteer, school, or vocational training program, and/or be employed.
  • Be able to use public transportation with minimal assistance.

Staff

A Residence Manager, Social Worker, Registered Nurse, and four Case Managers who function as a team.

Program

An Individual Program Plan (IPP) is developed for each person with goals in the areas of:

  • Daily Living Skills
  • Medical/Heath/Nutrition
  • Interpersonal Relationships
  • Vocational/Educational Skills
  • Recreation/Leisure Activities
