Sample Language for your WillFor an Outright Bequest of a Specific Dollar Amount “I hereby give, devise, and bequeath the sum of $_____ to the Epilepsy Foundation of Western Ohio, a tax exempt organization currently located at 7523 Brandt Pike, Huber Heights, OH 45424. This is an unrestricted gift and may be used at the discretion of its Board of Trustees to further the objectives and purposes of the Epilepsy Foundation of Western Ohio.” For an Outright Bequest of a Share, or Entire Residue, of Your Estate “I hereby give, devise, and bequeath ___% of my residuary estate to the Epilepsy Foundation of Western Ohio, a tax exempt organization currently located at 7523 Brandt Pike, Huber Heights, OH 45424. This is an unrestricted gift and may be used at the discretion of its Board of Trustees to further the objectives and purposes of the Epilepsy Foundation of Western Ohio.” Bequest to EFWO for a Specific Purpose or Program Please contact us at EFWO first to discuss the purpose or program you wish to designate so that we are best able to honor your wishes.
