Epilepsy Foundation Scholarship

The Epilepsy Foundation of Western Wisconsin is pleased to once again offer a $1,000 college scholarship for one graduate with epilepsy. The Foundation reserves the right to determine each year the number of scholarships to be awarded based on the quality of the applications. This award is not automatically renewed; you must submit a new application each year that you wish to be considered for the award.

Specific criteria for acceptance as a potential candidate are defined in the attached "Scholarship Fact Sheet".

 PLEASE DO NOT SUBMIT AN APPLICATION IF YOU DO NOT MEET THE MINIMUM REQUIREMENTS LISTED.

The scholarship recipient will be determined through a competitive scoring system that balances need with achievement. The winner will be notified of the award on or about November 16, 2008, and the award will be presented December 2, 2008.

Fact Sheet

Purpose: To assist an individual who has epilepsy with his/her academic and/or vocational training.

MINIMUM APPLICANT QUALIFICATIONS:

Applicant must:

1. Have a primary diagnosis of epilepsy/seizure disorder; provide name of physician and name of anticonvulsant medication.
2. Provide proof of acceptance to a post-secondary academic or vocational program.
3. Be a full-time student or equivalent thereof.
4. Submit a completed application no later than November 1, 2008.
5. Be a legal US resident 
6. Be a legal resident of or be attending a school in Western Wisconsin (our 22 county coverage area). (Click here to return to our homepage for a complete listing of our counties)

Note: Awards will be based on the quality of applications through a competitive scoring system that balances need with achievement. The Epilepsy Foundation reserves the right to determine each year the number of scholarships to be awarded based on the quality of the applications.
